The FMC Locator tool is designed to help you locate a physician, nurse or certified nurse midwife who has completed
the FMC program in your area for medical purposes.
The FMC Locator, or any of its contents, may not be downloaded, republished, resold or duplicated, in whole or in part,
for commercial or any other purposes including compiling mailing lists or any other list. The use of the FMC Directory
to establish independent data files or compendiums of statistical information is strictly prohibited. If you cannot
agree with these rules, do not use the Locator tool. The FMC program places "seed" names on the Locator to monitor
usage. These rules may be modified from time to time; the date of the most recent revisions will appear on this page.
Use of the Locator tool by you will constitute the acceptance of the User Agreement. Sanctions may include
identification of the violator's company / organization to FMC users.
